The Waiver

Release, Waiver of Liability, Defense, Indemnification and Hold Harmless Agreement By use of this equipment, you the user ("User" or "You) acknowledge that the donor (“Donor” or "We") is acting as a Good Samaritan and an independent volunteer providing dissemination of a 3D designed face shield. We do not certify the design or components operate properly or satisfy any regulatory requirements.

 This Release, Waiver of Liability, Defense, Indemnification and Hold Harmless Agreement (“Agreement”) acknowledges that Donor offers the design as an experimental device, without warranty of any kind. You are either the individual recipient of the design, if the individual recipient is an adult under the laws of the relevant jurisdiction, or the parent or legal guardian of the child if the individual recipient is a child under the laws of the relevant jurisdiction. You agree that the design provided to You by Donor or by any individual associated with Donor is provided “AS IS” and without representations or warranties of any kind, express or implied, and is intended to be a gift. By accepting the design, you further acknowledge that such design has not been evaluated or approved by any regulatory authority. You understand that the design furnished by Donor is meant to benefit society, and is not intended, and shall not be used, for commercial use. If You are an adult under the laws of the relevant jurisdiction and are the individual recipient, You acknowledge that You understand the potential risks of the use of the design and expressly assume such risks. If the individual recipient is a child under the laws of the relevant jurisdiction, You represent that You are the parent or legal guardian of the child and that You understand the potential risks of the use of the design and expressly assume such risks on behalf of the individual recipient. Furthermore, You (either for yourself or your child) release and forever discharge Donor from any and all liability for acts or omissions— including negligent acts or omissions—causing damage, loss, injury, or death to the individual recipient or others from the use of the design. You agree to defend, indemnify and hold Donor harmless from any and all liability or loss—including liability for negligence—arising in conjunction with or resulting from the individual recipient’s use of the design (including all attorney’s fees and expenses incurred by Donor). You further understand and agree that Donor and any individual associated with Donor shall not be liable for any injuries or damages, including any consequential, incidental, indirect, punitive or special damages, resulting from or arising out of the use of the design provided by Donor or any individual associated with Donor. Any information provided by Donor and any individual associated with Donor is not intended to be relied upon to protect from, diagnose or treat a health problem or disease, and Donor makes no claims regarding the design or the components. You should consider consultation with a qualified health care provider prior to utilizing face shields printed pursuant to the design. Any statements made by Donor are for informational purposes only and are not meant to replace the services or recommendations of a physician or other qualified health care practitioner. You acknowledge that this Agreement is governed by the laws of the State of Utah (without reference to its conflict of law provisions) and the parties intend that this Agreement be interpreted as broadly and inclusively as permitted by law. If any portion of this Agreement is found to be invalid for any reason, the remainder of the Agreement shall continue to remain in full force and effect and shall continue to be binding. By utilizing the design/face shield/mask, You acknowledge that You have carefully read the foregoing provisions, understand their contents, and agree with and to each and every provision herein.

Mask Inserts

To Whom It May Concern:

 

These masks are made from high quality materials that can be sanitized using one of the following methods; Hot soapy water, 70% or higher IPA, or bleach water. If using bleach water, please remove the filter, and allow the mask to soak at least 60 seconds or longer.  Each one of these masks were prepared in a clean sanitation station(wash, rinse, sanitize), but please re-sanitize them before using. A note of caution; these masks cannot handle the Autoclave. They will be destroyed and a big mess will await you upon opening the autoclave:) the highest heat they can handle is 145F. Please sanitize with IPA.

 

The material is not very porous, and if it was, it would not matter. IPA has a very low surface tension and it can get into spaces as small as viruses.

 

When washing and soaking, please remove the filter material first:) To do this, insert a pen or a pencil into the hole in the front of the mask. Push gently, and the filter clip will release from its housing. You can also sanitize the filter medium by baking it in an oven at 170 degrees F for 20 minutes. it will not melt since its glass transition temperature is higher than 170F. Whatever you do , do not get them wet, it will deactivate the static charge that is on the filter material that is used to act as a magnet to capture particles.

 

Once the mask has been washed and sanitized, replace the filter and clip back into place. The easiest way to do this is to grab the filter, center the clip onto the filter, so that there is some overhang, and then clip it into place.  Please refer to the picture below.

 

To cover the hard, plastic edge, either use 3M nexcare waterproof tape or some sort of silicone that is skin safe. I do not cover the edges with anything since I do not know whom is allergic to what.

Faceshield Inserts

These face shields are made from FDA approved material. They can be re-used, washed with hot water, soap and also sanitized with 70% rubbing alcohol, peroxide and or bleach water. The frame was made with a high-quality plastic designed for engineering applications and follows strict NIH guidelines for closed brimmed visors.

The face shield itself can be washed and sanitized as well. There are extra sheets supplied for each face shield kit. I have taken extra caution to ensure that these were sanitized, sealed, and put together in a clean environment. I am an engineer and scientist, who strives to help where I can while following health department standards.

This face shield requires bands to hold it in place. It fits tightly and comfortably around the head, while maintaining stability. There are 3 prepared NON-LATEX rubber bands in the kit. Use these to hook to the ends of the visor frame.

My procedure:
I have a sanitation station with two tables. All equipment I use is sanitized often to ensure a clean product. All personal whom helped, are required to wear gloves, a face mask, hair nets or have their hair pulled back and up.

Station 1 has a wash, rinse, sanitize sink system where all parts are cleaned and prepared for packaging. Once the part is finished with sanitization, it is IMMEDIATELY placed in a sterile, sealed bag, where it is left to sit for 2-3 days.

Station 2 is where all other materials required for the face shield kits, such as this instruction sheet, are placed into their own bags. This is to help keep the already sanitized parts clean😊
